Home Care Referral & Business Development Specialist (Commission-Based)

Position Summary

Circle of Care is seeking a motivated and relationship-driven professional to grow our home care services through community outreach and referral development. This role focuses on building trusted relationships and converting qualified leads into active home care clients.

This is a commission-based position designed for a self-starter with strong connections in healthcare, senior services, or the community.

Compensation Structure

Base Commission (per qualified client):

  • 15–20 hours/week: $250
  • 21–30 hours/week: $400
  • 31+ hours/week: $500

Performance Bonuses:

  • 3+ qualified clients in a month ? +$200 bonus
  • 5+ qualified clients in a month ? +$500 bonus
  • Commission is based on clients who start and receive services, not referrals alone
  • Unlimited earning potential based on performance

Commission Payment Terms

Commission is earned only when a referred client:

  • Is admitted to services
  • Begins receiving care
  • Is authorized for a minimum of 15 hours per week
  • Remains active and billing for a minimum of 30 days

Only clients meeting the above criteria count toward commission and bonus eligibility.

Key Responsibilities

  • Develop and maintain relationships with:
    • Nursing facilities
    • Hospitals and discharge planners
    • Social workers and case managers
    • Assisted living and senior housing communities
    • Community organizations
  • Educate referral sources about Circle of Care services
  • Identify and convert qualified individuals into home care clients
  • Support the intake process by ensuring smooth handoff of referred clients
  • Represent Circle of Care in the community and at networking events
  • Maintain communication regarding referral activity and outcomes

Performance Expectations

  • Generate qualified referrals that convert into active home care clients
  • Focus on securing clients with sufficient authorized service hours
  • Maintain consistent outreach and relationship-building efforts
  • Meet or exceed monthly client conversion goals

    • Home care
    • Healthcare marketing
    • Case management or social services
  • Strong communication and networking skills
  • Self-motivated, results-driven, and relationship-focused
  • Existing connections within healthcare or community settings preferred
